INVITATION TO PARTICIPATE IN A RESEARCH STUDY ON THE IMPACT OF COVID-19 ON WISCONSIN YOUTH AND FAMILIES

If you have a son or daughter between the ages of 6-17 and are living in the Wisconsin area, you are eligible for a research study conducted by the Department of Psychiatry. This study examines the impact of COVID-19 on the mental health and well-being of Wisconsin families. Youth between the ages of 10 and 17 have the option to participate in the study as well. Study participation entails completing a set of surveys that will take up to one hour each at three different time points.

We are inviting Wisconsin parents and caregivers of school-aged children between the ages of 6 and 17 to complete this research survey. There is an additional option for your child to also complete the surveys if they are between the ages of 10-17, however that is not required in order for you to participate. At this time, we are only able to accommodate one child to actively participate in the study.

This survey will take you up to one hour to complete. If you have an adolescent that is also interested in participating, they will complete the survey after you are finished. It will take them approximately 30 to 45 minutes to complete.

Due to the ongoing changes that COVID-19 has had on our community over the past months and the potential for continuing changes, we are requesting that you complete the survey three times: baseline (now), one-month follow-up, and six-month follow-up. The same adolescent must participate in all time points, if applicable. Participation in this survey at any time point is completely voluntary. You may choose to only complete one or two time points. You may choose to withdraw at any time.

For every set of surveys you complete, you will be given one entry into a weekly drawing for a $100 Amazon gift card. This means that if you complete the baseline, one-month, and six-month follow-up surveys, you could win up to $300 in Amazon gift cards. The same opportunities will be given to your adolescent, should they also choose to participate.

The same opportunities will be given to your adolescent, should they also choose to participate.
